FEES & SCHEDULING

Fees and scheduling

The fee is published here so you have it before you call. A conversation about whether an evaluation is the right thing is a better use of fifteen minutes than a conversation about what it costs.

Evaluation fees

ADHD Evaluation:

$2,200

Answers one question: is it ADHD.

Comprehensive Evaluation:

$3,500

ADHD and learning, with full achievement testing and documentation ready for accommodation requests. Reduced fees are available based on financial circumstances.

Students: $2,000 for the ADHD Evaluation and $2,750 for the Comprehensive Evaluation, with a current student ID.

Starting with ADHD: if the ADHD Evaluation shows a fuller evaluation is needed, you pay only the difference to move to the Comprehensive.

Included in Every Evaluation:

Concierge Assessment:

  • A direct line to Dr. Jovanovic, with a reply within one business day.
  • A 15-minute call before testing, so he knows your question and you know what the day looks like.
  • One unhurried morning, with breaks.
  • The report within 12 business days of the last record arriving, with a status update along the way.
  • A 20-minute call once you have read the report, to answer your questions.

Full Evaluation:

  • Testing
  • Scoring
  • Records review
  • The written report: Two-page diagnosis, full report behind it.
  • You also receive a letter written to you, in plain language, saying what the testing found and what it means. Results come in writing, in plain language, and a 20-minute call to answer your questions is included once you have read them.

Services beyond the concierge assessment and evaluation are billed separately at $300 per hour and agreed in writing beforehand. That includes attending a school or accommodation meeting.

Deposit and payment

A $1,000 deposit confirms the appointment. It is applied to the fee, not added to it.

The balance is due on the testing day, and the report is released once it is paid. Pay the full fee at booking and save 10%. One discount per evaluation.

Cancellation and rescheduling

Cancellations with at least three business days notice are refunded in full.

Cancellations with less than three business days notice, and appointments that are not attended, forfeit the deposit.

One reschedule with notice costs nothing.

Private pay

This is a private pay practice. Insurance is not billed.

An evaluation that is not scoped or limited by an insurer covers what the referral question actually requires, and produces documentation clean enough for outside reviewers to accept.

A superbill is provided on request, which you may submit to your insurer yourself. FSA and HSA cards are accepted.

Before you book

Preparation begins as soon as an evaluation is scheduled, and one part of it takes several weeks to assemble. See Preparing For Your Evaluation.

The report is delivered twelve business days from data-complete. Data-complete is the day the last record arrives, not the test date.

The report does not make recommendations on medication, dose or lab work.

Opens Monday, October 5.

Your Right to a Good Faith Estimate

You have the right to receive an estimate explaining how much your care will cost.

Under federal law, health care providers must give patients who do not have insurance, or who are not using insurance, an estimate of the expected charges for medical items and services.

You have the right to receive a good faith estimate for the total expected cost of any non-emergency items or services. This includes related costs such as testing, scoring, and report preparation.

You can ask any health care provider for a good faith estimate before you schedule an item or service.

If you schedule an item or service at least 3 business days in advance, your provider must give you a good faith estimate in writing within 1 business day of scheduling. You can also ask for one before you schedule.

If you receive a bill that is at least $400 more than your good faith estimate, you can dispute the bill.

Make sure to save a copy or a picture of your good faith estimate.

For questions or more information about your right to a good faith estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.

To request a good faith estimate, contact Dr. Dan Jovanovic at (941) 699-4797.
